1. Remote Patient Monitoring (RPM)
Connected devices: Bluetooth-enabled blood pressure monitors, glucometers, weight scales and pulse oximeters securely transmit your health data to your care team.
Data tracking: You or your caregiver log daily readings, which the pharmacy team reviews to spot early warning signs of complications.
Consultations: Your pharmacist reviews trends with you remotely and coordinates care or adjusts your plan directly with your doctor.
2. Medication Therapy Management (MTM)
Medication reviews: Pharmacists comprehensively review your prescriptions, over-the-counter drugs and supplements to identify side effects or dangerous interactions.
Medication organization: We organize medications into blister packs or compliance packaging so you take the correct dose at the correct time.
Medicine cabinet checks: In select local programs, pharmacists visit your home to safely dispose of expired medications and keep your list up to date.
3. In-Home Nursing & Wellness Checks
Hands-on care: We partner with Licensed Practical Nurses (LPNs) for in-home wellness checks, injections, and recording vital signs like heart rate and temperature.
